We are seeking a Patient Advocate in our Disability department to work in our regional office in Draper, UT. Full time with benefits. Various schedules available Mon - Fri 8am - 5pm or 9am - 6pm

By expediting successful SSI/SSDI applications we secure long-term payer sources for our hospital clients and monthly income and health insurance for the individuals we assist.

Primary
Responsibilities:
Collaborating telephonically with individuals who are potentially eligible for federal disability benefits.
Securing the information necessary to determine if the individuals meet Social Security's non-medical and medical criteria.
Providing empathetic, efficient and effective advocacy that results in an accurate assessment of eligibility and positive, productive relationships with the individuals with whom we come in contact.
Demonstrating a commitment to customer service.
Presenting a professional and informed demeanor.
Reviewing medical documentation.
Completing and forwarding all necessary forms and electronic documents.
Partnering with colleagues, hospital staff, case management, and family members to ensure that we gain contact with the individual and obtain the information necessary to help.
Solving problems and researching methods of overcoming roadblocks in the screening and application process.
Meeting productivity and performance standards requisite to success at Cardon Outreach.
Attending to various reports as instructed by supervisor.
Tracking and recording all account progression via Cardon Outreach technology platforms.
Attend, participate in, retain, and apply training provided by Cardon Outreach and the department
Excellent verbal and written communication skills, including the ability to understand others and effectively disseminate technical information.
Excellent customer service skills.
Ability to work independently and manage time effectively.
Experience in social work and/or case management.
A positive attitude that is flexible and adaptable to a changing environment.

Required computer skills: must have experience with data entry and word processing, be capable of operating routine office equipment, possess a working knowledge of MS Office applications, and understand how to navigate through web-based applications .
Degree desirable
Bilingual,particularly desirable but not necessary. English/Spanish

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
